For me, a first draft is when it's done - I've blurted out all I want to say and that's it. I don't leave notes of *insert whatever here* because I tend to write those to myself as I'm going along. No, I get what I want said, then I put it aside and don't look at it for a few weeks/months/years, then I go back, correct the spelling and grammar, and start shuffling things around for improvement.
